About the Program

What is CalAIM?

California Advancing and Innovating Medi-Cal (CalAIM) is a statewide initiative that expands Medi-Cal to fund non-medical community supports for members with complex or chronic health needs.

Rather than waiting until a crisis lands someone in the ER or a facility, CalAIM invests in keeping people safely at home — with consistent, practical support that reduces risk and improves long-term health outcomes.

For eligible Medi-Cal members, this means access to in-home care services that were previously unavailable or out-of-pocket — delivered by a vetted, authorized provider like Coast Care.

Eligibility

Who may qualify for CalAIM support?

CalAIM-aligned services are designed for Medi-Cal members whose needs put them at risk without consistent in-home support. If any of the following apply, we can help assess eligibility and coordinate next steps.

  • At risk of ER visits or hospital readmission
  • Transitioning home after a hospital, nursing, or rehab stay
  • Living with dementia, cognitive impairment, or complex chronic conditions
  • Needing consistent help with activities of daily living (ADLs)
  • Requiring supervision due to behavioral or health conditions
  • Benefiting from regular in-home monitoring and support
